Service Vendor Reporting Form

Service Vendors are required to report their Fair-time sales no later than November 30 each year. Reporting may be done by either:
1) Entering the report information below by Customer listing the sales by Product, Unit, and Quantity. Separate each field with commas.
2) Attaching a document with the sales information in the identified file upload area.
