Service Vendors are required to report their Fair‑time sales no later than November 1 each year. You’re welcome to submit your report in either of the following ways:

1. Enter your sales information directly in the form below by listing each customer’s purchases by Product, Unit, and Quantity, separating each field with commas.
2. Upload a document with your sales details in the designated file‑upload area.
